# Questionnaire

A Patient Questionnaire contains medical/dental questions (anamnesis) that can be also completed as a kiosk form (sent to a patient before visiting the clinic). [Profiles](/dentaltap/patients/profiles.md#patient-actions)

Patient Questionnaire can be configured with the predefined questions (a list with 100+ dental and medical questions), or custom questions in different formats - text, date, number, single choice, multiple-choice, etc. [Configure Questionnaire](https://jam.dentaltap.com/en/p/settings/templates/dictionary?id=questionnaire).

1. Open the Patient Profile and click on the **Questionnaire** menu item at the top.
2. Click on the **Edit Mode** to start editing the medical/dental questionnaire. Once it's edited, you will see the **Save Changes** button at the top. (Don't forget to click on it to apply your changes).

### Configure Questionnaire

Open the link [Configure Questionnaire](https://jam.dentaltap.com/en/p/settings/templates/dictionary?id=questionnaire) to edit your medical/dental questions.

1. Click on the New Field button to add your own field - date, text, single choice, multiple-choice.
2. Click on the From Library button to select fields from our library (100+ medical/dental questions).
3. Use the dots icon for each row at the left to arrange questions by index (click and drag to change the position index).
